Community Needs Assessment & Health Improvement Plan (2024-2025)

We are conducting a Community Needs Assessment (CNA) for 2024-2025 to better understand the health needs, social service needs, and well-being of Middlesex County residents. This assessment will help identify community strengths, gaps in services, and areas for improvement to ensure everyone has access to the support they need.

In addition to the community needs assessment, the findings will guide the development of a Community Health Improvement Plan (CHIP), which will focus on practical solutions to improve health, address barriers, and create lasting positive change.

The Community Health Improvement Plan (CHIP) is a strategic framework that identifies priority health issues and strategies to address the complex and persistent health challenges that affect all residents. In Middlesex County, the Governmental Public Health Partnership (GPHP) and Office of Health Services engages local health departments, hospitals, community service providers, local businesses and other partners to assess community and clinical linkages and coordinate resources. The Community Health Improvement Plan (CHIP) helps to guide agencies in creating new policies and aligning programs and services that define a vision of health within Middlesex County.

The top priority health issues identified through the CHIP process were (but are not limited to):

  • Access to Health Care and Information
  • Coordination of Communication among Community Partners
  • Promoting Healthy Behaviors
  • Disease Specific Issues: Focus on Obesity, Diabetes and Mental Health
