Voter Registration for Middlesex County (Should also be used for Change of address, Name Change or Party Change):
DO NOT E-MAIL THIS FORM – Complete the form on your PC, print it out, sign it and send or bring it to the Middlesex County Board of Elections or your local Municipal Clerk’s office.
Mail-In Ballot:
If you applied for a Vote by Mail ballot but lost or misplaced it, if there is enough time, you may request another ballot at the County Clerk’s office. If there is not enough time to get a new Mail in Ballot, you can still vote at your polling location. However, you may not vote on the machine. You must vote by Provisional Ballot. Once you apply for a Vote by Mail Ballot, the Poll Books will be stamped with “Mail in Voter” where you would sign. The Poll Worker is required to have you fill out a Provisional Ballot when that occurs. Don’t worry though, your vote will still count just as you would have voted by mail or on the machine.
